Mentoring Self-assessment

You are invited to participate in the National R3 Implementation Workgroup's Mentoring Program Self-assessment. The purpose of this self-assessment is to serve as a tool that R3 practitioners can use to improve their programs. This self-assessment can be taken as many times as needed. If your organization runs more than one mentoring program, please take a separate assessment for each program.

As you go through, refer to this page for key definitions. We recommend having this page open as you go through the assessment.

This self-assessment is comprehensive and may take up to 30 minutes to complete. You can leave the page and pick up where you left off from the same computer. Throughout the assessment, you will find resources to help you craft an effective mentorship program. These resources can also be accessed after completion of the self-assessment through the National R3 Clearinghouse.

Your participation in this self-assessment is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the assessment at any point. It is very important for us to learn about your mentorship efforts. Your self-assessment responses will be strictly confidential and data from this research will be reported only in the aggregate.

If you have questions at any time about the survey or the procedures, you may contact the Council at Please start with the survey now by clicking on the Next button below.
